Paste Text Only button on Quick Access Toolbar is sometimes disabled

i inserted command "paste , keep text only" on quick access toolbar.  active , disabled (even when there text in buffer).  can't determine how make active when there .  likewise shortcut keys (alt+shift+v) work sometimes.  i've found if drop dropdown paste option down , hover on paste options, button becomes active (for session of word).  can go out of word and come in , it's active, becomes inactive again.  there cure copy something, paste text open active?

generally, paste options enabled depends on formatting on clipboard, seem strange paste , keep text isn't active. make use of macro pastes in plain text format:

 

sub pasteplaintextformat()
on error resume next
selection.pastespecial wdpastetext
end sub

i'm not familiar alt+shift+v shortcut. 1 assigned? if so, verify stored in correct template (usually want in normal template). the default shortcut pasting ctrl+v.
