"a required subref is missing" --> error message, on ADAM command prompt, csvde -i -f input.csv

i getting same error message --> 'a required subref missing' 
whenever try import  csv file adam. 

i run command in adam command prompt.  

c:\windows\adam   dsvde -i -f   input.csv

my input file  looks ===>   attributes on top row & value on 2nd row.
attribute                             value  

objectclass                        user
samaccountname           petergr
dn                                   cn=peter graham,ou=adam users,o=microsoft,c=us    
displayname                     peter graham
useraccountcontrol          514

is problem the   adam  adsi edit  ??   or problem input file??
please , on why error message keeps poping up
